We provide a full range of services, including Companion Care, Personal Care, Light Housekeeping, Assistance with Daily Living, and Meal Preparation. Our goal is to support your loved one’s independence, comfort, and well-being.
Gracie’s Home proudly works in partnership with Alta California Regional Center (ACRC) to provide residential care for children with complex support needs. All placements are coordinated through ACRC to ensure the right level of care and support for each child.
If you believe Gracie’s Home may be a good fit, the first step is to connect with your ACRC Service Coordinator. They can help determine eligibility, discuss placement options, and submit a referral to our home if appropriate.
Once a referral is received, our team works closely with ACRC and the family to review the child’s needs and plan for a smooth, thoughtful transition.
Please note: Gracie’s Home does not accept direct placements. All admissions must be initiated and approved through Alta California Regional Center.
